PRIMARY CARE NETWORK
Due to changes in the GP Contract, groups of practices are now working together - typically covering between 30-50 thousand patients in an area.
Primary Care Networks build on a core of current primary care services and enable greater provison of proactive, personalised, co-ordinated and more integrated health and social care services.
Primary Care Networks provide the basis for future collaboration with other providers (eg Community Trusts) where appropriate, and will work together to deliver specific packages of care.
