NEW PATIENTS

Our list is open. The information on this page will enable you to understand how to register as a patient at this practice and give information on the rights and responsibilities of patients.

When registering please bring your NHS Medical Certificate to the practice and those of any other persons registering with you.

If you do not have a medical card, you will be asked to complete a registration form for each person registering with the practice.

In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period while your medical records are transferred from your old practice to this one.

If you move out of the practice area, you will need to register with a new practice as soon as you move. We can provide information about how to find a new practice.

In order to register at our surgery you will need to complete the following documents:

  • New patient Questionnaire
  • GMS1 Form
  • Application Form for online services

Please bring the completed documents with some form of identification - preferably a utility bill, driving licence or passport to the surgery and our admin team will start the registration process for you.

Each patient has a named accountable GP for the purposes of clinical oversight, but you may request an appointment with any of our doctors.

Register at the surgery

You can register with a GP in England regardless of your nationality or immigration status.

We will not ask about your immigration status and your information is confidential. If you are worried about giving us your address, please let us know.

It does help us to get accurate details if you can bring some form of photo identification such as a passport, driving licence or Home Office ID and proof of current address. Please come and talk to or call the Manager on duty if you have any questions or concerns about registration.

You will need to bring in a print out of your current repeat medications. Please note also that we can arrange an interpreter if you have issues understanding and communicating in English.

You will be asked to complete a registration form called a GMS1.

Many migrants will come from countries with very different health care systems to the UK.

It will almost certainly be helpful to explain to new patients how the NHS operates and explore with them how this compares to the healthcare system that they've been used to.

Accessible Information Standard

The Accessible information standard (AIS) is an NHS England information standard which must be implemented by all organisations that provide NHS or adult social care.

The AIS aims to ensure that people who have a disability, impairment or sensory loss receive information that they can access and understand - for example in large print, braile, professional communications support if they need it , such as a British Sign Language interpreter.

We want to get better at communicating with our patients. We want to make sure you can read and understand the information we send you. If you find it hard to read our letters or if you need someone to support you at appointments, please let us know:

If you need information in braille, large print or easy read. If you need a British Sign Language interpreter or advocate. If we can support you to lipread or use a hearing aid or communication tool

Please tell the receptionist when you arrive for your next appointment, or call us. This information will be recorded on your medical record in a standardised way and highlighted to ensure we have information on your communication needs.

This information may be shared with other NHS and adult social care providers if required.

If you require a specific method of communication, please inform the practice as soon as possible to ensure this information is recorded.

Temporary Residents

If you are staying in the Beverley area, we are happy to see you in our normal surgery appointments, either with doctors or nurses. You will be asked to complete a simple form, recording your own doctor as well as your permanent or temporary address. If you are staying more than 3 months, you should register as a permanent patient. A copy of our temporary resident form can be downloaded and printed from the link below.

Visitors from abroad are welcome. European and Commonwealth residents can expect full access to free NHS emergency care. Some countries do not have such arrangements with the NHS, and so patients can expect to be charged for consultations.

As a temporary patient, we will not have access to your medical records so will be restricted in what log-term care we can offer. This will be discussed with you at your appointment but is usually best left to your own doctor.

Equality & Diversity

The Practice is committed to eliminating discrimination, harrasment and victimisation. We aim to advance equality of opportunity and foster good relations between different parts of the community
