Social Prescribing and community-based support is a key component of personalised care and is part of the NHS Long Term Plan.
Social Prescribing enables GP practices to refer patients to a range of services that offer support for social, emotional or practical needs. Through a link worker, social prescribing connects patients to community groups and services.

Depending on which service you require, the team will be in touch with you following a referral from your usual GP practice. You will be seen either face to face or over the phone, where you needs will be assessed and you can set some achievable goals to work towards. You will then be seen or contacted regularly by your clinician and you can discuss any progress you've made with your goals as well as any set backs you may have faced.
